Apicectomy or re-root canal?

Where the existing root filling is poor, retreatment through the crown is usually the first choice — it addresses the whole canal system. Surgery is preferred when there is a well-fitting post and crown, a blocked or ledged canal, or a fractured instrument in the way.

What happens during surgery

  • A small flap is raised to expose the bone over the root tip
  • Infected tissue is removed and sent for analysis if indicated
  • The last 3 mm of root is trimmed away
  • The root end is prepared and sealed with a biocompatible material
  • The flap is repositioned with fine dissolvable sutures

Success and healing

Modern microsurgical apicectomy with magnification has a success rate of around 85–90 per cent. Swelling and bruising settle over three to five days; bone healing shows on X-ray over six to twelve months, and we review it with follow-up radiographs.
