Do wisdom teeth always need removing?

No. NICE guidance is clear that healthy, symptom-free wisdom teeth should be left alone. We recommend removal when there is repeated infection, decay that cannot be restored, cysts, damage to the tooth in front, or pain that keeps returning.

Simple versus surgical removal

  • Simple — a fully erupted wisdom tooth is lifted out much like any other tooth
  • Surgical — an impacted tooth needs a small gum flap, sometimes sectioning the tooth, then dissolvable stitches
  • Lower impacted teeth take longer than uppers and swell more
  • Complex cases close to the nerve are referred for CBCT and specialist care

Recovery, honestly

Expect swelling that peaks at 48 hours, some jaw stiffness and a week of soft food. Most people take two to three days off strenuous activity. Ice for the first day, warm salt rinses from day two, no smoking and no straws — that combination prevents most dry sockets.
